RENEGADES GIRLS FLAG FOOTBALL — PARTICIPANT WAIVER, RELEASE OF LIABILITY, AND INDEMNIFICATION AGREEMENT

Season: 2025–2026 Season | Organization: Renegades Girls Flag Football | Location: Metro Atlanta, Georgia

1. ACKNOWLEDGMENT OF RISKS. I, the undersigned parent or legal guardian ("Parent/Guardian"), on behalf of myself and the minor athlete named above ("Athlete"), acknowledge that participation in flag football and related activities organized by Renegades Girls Flag Football ("the Organization") involves inherent risks of injury, including but not limited to: sprains, strains, fractures, concussions, and other physical injuries; risks associated with travel to and from events; risks associated with outdoor athletic activities including weather-related conditions; and any other risks inherent in competitive athletic programs.

2. ASSUMPTION OF RISK. Parent/Guardian voluntarily and knowingly assumes all risks associated with the Athlete's participation in all Renegades activities, including practices, clinics, camps, showcases, tournaments, travel events, and any other program activities, whether on or off the field. This assumption of risk applies regardless of whether such risks arise from the negligence of the Organization, its coaches, staff, volunteers, or other participants.

3. RELEASE AND WAIVER OF LIABILITY. In consideration of the Athlete's participation in the Renegades program, Parent/Guardian, on behalf of themselves, the Athlete, and their respective heirs, executors, administrators, and assigns, hereby releases, waives, discharges, and covenants not to sue the Renegades Girls Flag Football organization, its directors, officers, coaches, staff members, volunteers, sponsors, and affiliated organizations (collectively, "Released Parties") from any and all claims, demands, losses, damages, costs, and expenses arising out of or in connection with the Athlete's participation in any Renegades activity, including claims arising from the negligence of the Released Parties.

4. INDEMNIFICATION. Parent/Guardian agrees to indemnify and hold harmless the Released Parties from any and all claims, actions, damages, liability, and expenses in connection with loss of life or personal injury, or damage to property, arising out of or resulting from the Athlete's participation in Renegades activities, or from the Parent/Guardian's own conduct at any Renegades event.

5. MEDICAL AUTHORIZATION. In the event of an emergency in which the Parent/Guardian cannot be reached, Parent/Guardian hereby authorizes the Organization's staff to consent to any necessary medical treatment for the Athlete, including emergency medical care, hospitalization, surgery, or other procedures deemed necessary by medical professionals. Parent/Guardian agrees to be responsible for all medical costs incurred.

6. CODE OF CONDUCT. Parent/Guardian agrees that the Athlete will abide by the Renegades Code of Conduct at all times, including during practices, games, travel, and any team-related activities. Parent/Guardian also agrees to abide by the Renegades Parent Code of Conduct. Violations may result in suspension or removal from the program without refund.

7. PHOTO & MEDIA RELEASE (OPTIONAL). Unless declined below, Parent/Guardian grants the Organization permission to photograph and/or video record the Athlete during Renegades activities and to use such images and recordings for promotional, educational, and social media purposes without compensation. Parent/Guardian may decline this release by unchecking the media release option in this form.

8. SEVERABILITY. If any provision of this Agreement is held to be invalid or unenforceable, the remaining provisions shall continue in full force and effect. This Agreement shall be governed by the laws of the State of Georgia.

9. ENTIRE AGREEMENT. This Agreement constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior agreements, representations, and understandings. Parent/Guardian acknowledges that they have read this Agreement in its entirety, understand its terms, and are signing it freely and voluntarily.
